24-Hour Residential Group Homes
Haven Center of Oregon provides 24-hour residential supports for adults with intellectual and developmental disabilities in small, home-like settings designed to promote safety, dignity, choice, and meaningful community inclusion.
We operate several licensed residential homes in the Beaverton area, with each home typically supporting 3 to 5 residents. Professionally trained staff are present 24 hours a day, 7 days a week to provide individualized supports based on each person’s needs, preferences, and goals.
Supports may include assistance with:
Activities of daily living and personal support needs
Medication administration and health-related supports
Skill-building for greater independence
Behavioral and emotional support, as applicable
Transportation to appointments, activities, and community outings
Support with recreation, relationships, and community participation
Our homes are designed to be more than care settings, they are homes where individuals can build routines, pursue interests, develop skills, and receive support in a stable, person-centered environment.
As a licensed provider, our homes are subject to ongoing oversight and monitoring by state and county authorities, and we are committed to maintaining quality services that meet both regulatory standards and the needs of those we support.
APD Supports
Haven Center of Oregon provides residential supports for older adults and adults with physical disabilities who need assistance with activities of daily living in a supportive home environment.
Our APD supports are designed for individuals who may benefit from help with daily routines while maintaining as much independence, comfort, and choice as possible. Services are individualized based on each person’s needs and preferences.
Supports may include assistance with:
Personal care and activities of daily living, such as bathing, dressing, grooming, and mobility support
Medication assistance and health-related monitoring
Meal preparation and nutritional support
Household support and daily routines
Appointment coordination and transportation support
Social engagement and community inclusion
Support related to aging, chronic health conditions, or physical limitations
Community Living Support Services
(DSA Program)
At HCOO CLS, our day program engages individuals in structured, person-centered group activities designed to support daily living and community inclusion.

Community Engagement: Regular trips to local parks, libraries, and events to build social confidence.
Life Skills: Hands-on practice with money management, basic meal prep, and transit navigation.
Health: Adaptive fitness and group dance activities,
Mental Health and Wellness: Symptom management and practical coping skill workshops
Creative Arts: Art and music therapy, bracelet making, and karaoke
Community Outings
Discovery: OMSI, Oregon Zoo, and the Tillamook Cheese Factory
Nature: The Grotto, Hagg Lake, and Seaside beach trips
Seasonal: Tulip & Peony Festivals and the Pumpkin Patch
Recreation: Local bowling in Beaverton and community park tours
Supported Living Services
Supported Living Services are designed for adults with intellectual and developmental disabilities who want to live more independently in their own home or apartment, while continuing to receive individualized supports based on their needs.
Unlike 24-hour residential services, supported living does not involve around-the-clock staffing in the home. Instead, supports are tailored to the individual and may include scheduled check-ins, skill-building, and assistance to promote health, safety, and greater independence.
Our approach is person-centered and focused on helping each individual strengthen skills, maintain stability, and exercise greater choice in everyday life. Supports are developed around the person’s goals and can be adjusted as needs change.
